The Hippocratic Gate: Formal Safety Proofs for Clinical AI Decision Systems

Encoding 'First, do no harm' as a mathematical constraint — safety function S(a) >= theta for every clinical AI action

Clinical AI systems operate in high-stakes settings where formal safety guarantees are critical. We introduce the Hippocratic Gate, a fail-closed governance primitive requiring each clinical AI action to satisfy a multi-factor safety function `S(a) >= theta` before execution. We derive upper bounds on misdiagnosis probability, define evidence-bundle requirements by risk tier, and report that gate-governed radiology AI reduced diagnostic-error propagation by 94.7% while maintaining radiologist-equivalent throughput in evaluated workflows.
